New Patient Registration
Patients wishing to register can come to the medical centre and pick up a registration pack, alternatively the GMS1 form (registration form) can be downloaded from our “Registering Online” page.
When completed please hand the forms to the receptionist with two items of identification, these should be:-
- Proof of address - utilities bill / letter from landlord / Rent book (no bank statements or bank cards please)
- Photographic ID - Passport / Driving licence / student card.
Registration will normally take 48 hours after which you will be invited to make an appointment with the nursing team for a new patient health check. It should be noted that there is normally a 6 - 8 week period before the medical centre receives patient's medical records from their previous medical centre.
We register patients who have a postcode that begins with PO1, PO4 or PO5.
If a patient has moved while registered with us they are able to stay registered with the medical centre as long as they live within the PO1, PO2, PO3, PO4 or PO5 area. If a patient moves off Portsea Island they will be required to register with different medical centre.
The catchment area for this practice is PO1, PO4, PO5.
Note: Unfortunately should you move off Portsea Island you will have to register with another doctor closer to your home.
PLEASE KEEP US INFORMED OF ANY CHANGE OF NAME, ADDRESS OR TELEPHONE NUMBER SO THAT WE CAN CONTACT YOU SHOULD THE NEED ARISE.
Most of our clinical areas are based on the ground floor and should be easily accessed by disabled patients. The main doors are automatic to enhance access. Interior door are not automated. A loop system is available to help those with hearing aids.
As part of the new contract between the department of health and GPs nationally, we are now required to allocate an individual named GP to each patient. Your named GP is a lead GP who is the coordinator of your care but is not required to deliver all the care to the patient, nor expected to be available 24/7.
Having a named GP does not mean that you have special access to that particular GP. Additionally you will continue to see which ever GP you prefer.
If you are unhappy with the GP allocated to you we will make every effort to allocate you the GP of your choice however this may prove difficult if your preferred GP already has a large list size.
